The Side by Side Skill Development and Coaching Program offers an interdisciplinary care team of Mental Health Providers, Skills Coaches, and Peer Mentors who walk side-by side with our Members “moving as one” to help them build life skills, manage emotions, and connect with others through personalized coaching and lived-experience peer to peer support.

BridgeBack is designed to reduce barriers to safe and timely discharges. Our Bridge Guides specialize in helping seniors and high-need adults move from hospitals, nursing homes, and other acute settings into stable community living. We handle the details that delay discharges - housing, paperwork, benefits, and transportation so providers can focus on care.
